Definitions of terms used across NDIS, aged care and DVA documentation. Terms defined by a regulator link to the regulator’s page. Terms marked as working definitions describe how NoteGate uses the word.

Shift note
Working definition: a record written by a support worker during or at the end of a shift describing the support delivered, what the participant did and said, and anything the next worker needs to know.
Progress note
Working definition: a record that summarises a participant’s progress towards their goals over a period, usually drawing on several shift notes. See the guide to shift notes and progress notes.
Reportable incident (NDIS)
An act or event that has happened, or is alleged to have happened, in connection with delivering NDIS supports or services and that a registered provider must notify to the NDIS Commission, such as death, serious injury, abuse or neglect. NDIS Commission.
Incident management system
A dedicated set of processes and procedures that set out the actions and responsibilities of workers, the registered provider and other stakeholders when an incident occurs. NDIS Commission.
Serious Incident Response Scheme (SIRS)
The aged care scheme under which providers must notify the Aged Care Quality and Safety Commission of reportable incidents. There are 8 reportable incident types, split into Priority 1 (24 hours) and Priority 2 (30 days). Aged Care Quality and Safety Commission.
Behaviour support plan
A comprehensive or interim plan that documents positive behaviour support strategies for a person with disability and may include restrictive practices. NDIS Practice Standards definitions.
Restrictive practice
Under section 9 of the NDIS Act 2013, any practice or intervention that has the effect of restricting the rights or freedom of movement of a person with disability. Regulated restrictive practices are subject to authorisation and reporting requirements. NDIS Commission.
Unauthorised restrictive practice
Use of a restrictive practice that is not authorised by the relevant state or territory or does not follow a behaviour support plan. It is a reportable incident, notified within 5 business days. NDIS Commission.
NDIS Practice Standards
The quality standards that registered NDIS providers must meet, organised into a core module, supplementary modules and a verification module, each with outcomes and quality indicators. NDIS Commission.
Group 0138 (supported independent living)
The registration class for assistance with supported independent living. The NDIS Commission states mandatory registration and the SIL Practice Standards apply from 1 July 2026, with transition pathways. NDIS Commission.
Verification audit
An audit for providers delivering lower risk or lower complexity supports, completed as a desktop review of required documentary evidence by an approved quality auditor. NDIS Commission.
Certification audit
An audit for providers delivering higher risk or more complex supports, with a Stage 1 desktop audit and a Stage 2 onsite audit. NDIS Commission.
Non-conformity
A gap an auditor identifies against the standards. The NDIS Commission notes that a non-conformity does not mean a provider has failed; it means something needs to be corrected or strengthened within a timeframe. NDIS Commission.
Strengthened Aged Care Quality Standards
Seven standards that applied under the Aged Care Act 2024 from 1 November 2025: the individual, the organisation, the care and services, the environment, clinical care, food and nutrition, and the residential community. Aged Care Quality and Safety Commission.
DVA Gold Card and White Card
DVA health cards. Community nursing is available to Gold Card holders with an assessed clinical need and to White Card holders for accepted conditions. Department of Veterans’ Affairs.
Evidence chain
Working definition: the linked record of a note or incident from first draft, through checking and correction, to the final saved version, so an assessor can follow what was written, what was flagged and what changed.
