A plan reassessment turns on whether the supports delivered can be shown to have made a difference. That evidence exists in daily documentation or it does not exist at all, and by the time anyone checks, the period being reviewed is over. Nothing can be added to it that is not already there.
The common failure is not a missing report. It is discovering, in the meeting, that six months of notes recorded what happened without ever recording how the participant was doing relative to their usual baseline. There is nothing to compare, so there is nothing to argue from.

It refuses to generate a package it cannot stand behind.
Before assembling anything, NoteGate checks the evidence underneath. It flags missing baseline comparisons, absent formal assessments, and incidents that were never risk classified. If no note in the period records a baseline comparison at all, generation is blocked outright. A package that looks complete and is not is worse than no package, because it is taken into a meeting and relied on.

NoteGate does not write your submission, does not decide whether a support was reasonable and necessary, and does not generate a clinical fact that a worker did not record. Goal coverage is a count of the notes citing each goal, not an argument that the goal was met.
That boundary is deliberate. Evidence a coordinator can stand behind is evidence that came from the record, not from a system inferring what probably happened.
